This is a pretty simple financial model. There are really only 3 numbers that you need to enter into the worksheet. Those entries are highlighted in the color yellow.
Earning Per Share can be found on the income statement of the company. I use Yahoo finance for this.
You will need to estimate the growth rate. Personally I like to use Yahoo here again. Go to the analysis page, and it shows the estimated growth rate for the next 5 years.
